Step 1: Register
In order to register for my services, please email me at maria@benefitiaforms.co.uk to arrange our pre-assessment 45 minute phone or video call. The purpose of this is to gather vital information to begin working on your application and discuss what evidence you are going to submit. Following this, I will make a further appointment with you to meet via phone or video call, in order to fill out the main body of the form with you.
If you are applying for PIP and would like me to write your form for you. You will need to send your form to me by special delivery (where electronic copies of the PIP form are not available) along with all your supporting evidence.
Step 2: Collating Evidence & Preparing for your Appointment

It is important that you send all your evidence (test results, medical reports, EHCP etc) to me no more than 48 hours before the consultation, so I can review this alongside the information that will be contained in your form. The most convenient way to do this is via a file sharing platform. Alternatively, you can send copies to me by post, preferably by special delivery. If you require your evidence to be returned by post, I am happy to do this if you cover the cost.
Step 3: Phone or Video Appointment

I offer consultation both over the phone or by video call to enable me to write your form. This can last up to three hours, depending on the level of information that needs to be documented and breaks can be arranged. It is best if you are at home or in a quiet environment and you have a reliable internet connection.
During the consultation, I will take you through every section of the form, ensuring that I document your responses in detail. I will also record the consultation, so I can refer back to it when filling out the form (the recording will be deleted after your form is completed). Please note that I am not able to provide you with any legal advice outside of that which is published by the government. If I need any further information after the consultation, I will request this by email.
Step 4: Form Writing and Final Draft

Following our appointment, a draft copy of your form will be sent to you by email within seven working days. If there are any changes you would like to make or extra information that you would like to add, please email these to me as soon as possible. Once I have sent you the draft copy please read it thoroughly. If you are happy that it is a true representation of your situation, I will email your completed form to you in the case of DLA and Attendance Allowance and return your completed PIP form and all evidence sent by special delivery. You can then submit your form to the DWP.

Step 5 : How to prepare for Assessment Guide (PIP only)

This free E-guide is a downloadable resource and will be emailed to you once your PIP 2 Form is complete.
It is a valuable resource which will help you to prepare for your assessment. It covers preparing lists of medication and any aids or appliances that you might use. It will also provide you with vital information about the different types of assessment methods and what to expect during the assessment, including reminders about the “reliability criteria” and the “50% rule”. This guide will give you the confidence that you need to attend your appointment and be able to give detailed answers that demonstrate which descriptor best matches your difficulties in the 12 everyday living PIP activities.
